The form on this page is a free Spark sample request. These sentences are the offer. The form below is how you send it.
The form starts with two choices: "I'm an educator" or "I'm a program administrator." A later step asks for a role: Youth Worker, Educator, Librarian, Program Administrator, Director / Supervisor, or Principal / CEO. It also asks which organization you represent, including a public school, independent school, youth program, library, government agency, university, scout troop or organization, homeschooling family, or microschool.
Educators are asked, "Which one of our most popular Sparks do you want to sample?" Program administrators are asked, "Which sample pack would you like to receive?" The pack step says, "One box, packed for the grades you serve." The form's pack choices are Lower Elementary Sample Pack, "A set of Sparks for grades Pre-K - 2"; Upper Elementary and Middle Sample Pack, "A set of Sparks for grades 3 - 8"; and All Sparks Sample Pack, "A collection of eight Sparks serving grades Pre-K - 8." The educator path does not state how many pieces are in the Spark you pick.
Before you can submit, the form requires this acknowledgement: "I agree that FutureMakers may send updates to the email address I provided, and I understand that free sample requests are limited to one per requester."
The how to buy page says a free sample from this page is "shipped via UPS at no cost." The form's last step is titled "Where should we ship your Spark?" Country choices are the United States and Canada. It asks for a street address and a phone number labeled "Phone Number (Required by UPS)." After you submit, the form says you will get an email confirming the request and another email when it ships.
